Heavy Eyelids
As we age there is a reduction in the amount of collagen and elastin in our skin. Collagen provides skin with its strength. Elastin provides skin with elasticity and allows it to snap back when we are young. The loss of both these integral components in the skin causes our skin to become sallow and wrinkle and ultimately sag. In the face, as we age, there is also a loss of volume of the fat below our skin as well loosening of ligaments that hold all our facial structures tightly together. These all account for a sagging effect that means we end up with nasolabial folds, deep tear troughs and also droopy heavy upper eyelids.
What causes our upper eyelids to droop?
Aside from the loss of collagen and elastin in our skin our upper eyelids sag and droop with age due to loss of volume in the fat compartments within the temples and above the eyebrows. The areas around the temples and above our eyebrows hollow out with age and as a result the skin over this region tends to loosen resulting in a sagging effect of the upper eyelids. Any treatment to improve the upper eyelids need to tighten up the skin within the upper eyelid but also needs to address these regions of volume loss as well.
Sylfirm X for Droopy Eyelids
Sylfirm X is a microneedling device with needles that emit Radio Frequency Energy. It is very similar to Morpheus 8. The difference, however, is that Sylfirm X uses a Pulsed as well as a Continuous wavelength for the radio frequency. This means it can be used in the treatment of droopy eyelids. The pulsed radio frequency wavelength setting enables Sylfirm X to target delicate loose skin in the upper and lower eyelids without damaging adjacent soft tissues. Morpheus 8 cannot treat and tighten this area. The effect is a non-surgical blepharoplasty. Sylfirm X is generally well tolerated with a very short downtime. Treatment would require more than one session with significant results often evident after 3 sessions.

Treatments for heavy eyelids can be considered to cause direct and indirect effects. Rejuvenation of the eye area involves targeting and improving structures around the eyes as well the upper eyelid itself.
The gold standard for treatment of heavy upper eyelids is a blepharoplasty. This is the surgical removal of excess skin from the upper eyelid. Although relatively simple in principle a blepharoplasty is an extremely intricate procedure as the anatomy of the eyelid is very delicate and a miscalculation in the amount of skin being taken off can result in the eye not shutting properly and ultimately significant consequences.
A non-surgical blepharoplasty involves less invasive treatment to tighten up the skin of the upper eyelid. This is normally done using a plasma pen. Plasma is considered to be the fourth state of matter and the energy produced is the result of an electric charge igniting gas – in this case oxygen. The energy released can be used to make small scores in the upper eyelid that are extremely superficial. As these heal then tend to have a tightening effect on the skin of the upper eyelid and help to lift it. It can be performed using just topical numbing cream. There is some downtime associated with this procedure and there can be some swelling postoperatively.
Hyaluronic acid dermal fillers can be used to revolumise areas around the temples and just above the eyebrows in order to rejuvenate this region and improve overall head and face shape. This has the indirect effect of pulling on the upper eyelid and making it appear less droopy.
